Stormberg Orthodontics awards scholarships each year to graduating seniors, who are current or former patients of Dr. Kurt Stormberg. Applicants must be a student in grade 12 for the current school year. Applicants must submit a copy of their Senior Photo. Separately, applicants may list any additional information about them, including special achievements or activities, hobbies, talents, or anything they believe might be useful to Stormberg Orthodontics in evaluating their applications. Applicants may also list extracurricular school and/or community activities in which they participate. Applicants must describe their role in each activity.
